FYI: don't I feel stupid! Someone posted the url to the book last week. I
was about to print it off (the whole 1000 pages - I had even bought a spare
laser cartridge for when the first one runs out), and then decided to check
amazon at the last minute.
The hardcopy book is available at www.chapters.indigo.ca and is probably
available at amazon. Shipped in 24 hours, got it a few days later.
